Maybe one who lives at home with parents, a lonely cat lady, a recluse. My 60-year old neighbor is an unmarried woman (she may be lesbian, I don't know). I'd never think of her as an "old maid" because she's attractive, fit as fuck, travels a lot, is vivacious and social. She has a great life. There are other women I might think of as "old maids", but I can't think of any. Well, some older ladies we knew when I was a kid. And now that I think about them, they were either quietly lesbian or slightly autistic. My sister is extremely attractive and 45-looks-25. She never lacks for male attention, but she's got some personality disorder that keeps her single. An "old maid" is an old virgin. That's not what an old maid means. It means a woman who reached middle aged without getting married and is now destined to be alone because she "missed her chance" when she was in her prime. Another word for this was spinster.
